We accept members transitioning from various inpatient settings, including hospitals, skilled nursing facilities, residential treatment facilities, and behavioral health centers. To ensure a supportive environment, we require residents to be self-sufficient, capable of managing their activities of daily living (ADLs), and motivated to pursue healing and a brighter future.
We proudly offer 20+ homes across Riverside and San Bernardino County, with luxury amenities such as 3 square meals a day, ADU accessible homes, Nurses, 24/7 staff, full service internet, washer/dryer, telephone, and large TVs in every room at no additional cost to members.
We accept referrals from hospitals, skilled nursing facilities, treatment facilities, and behavioral health centers. Eligibility is determined on a case-by-case basis, considering medical needs, behavioral stability, and our ability to provide appropriate care. We serve adults 18 and over who need supportive services.
Length of stay varies based on individual needs and program type. Recuperative care stays typically range from 30-90 days, while supportive housing can be longer-term as residents work toward independence. Our case managers work with each resident to develop appropriate transition plans.
Costs vary depending on the program and funding source. We work with various payment options including Medi-Cal, private insurance, county contracts, and grant funding. We have 25+ homes across four Southern California counties: Riverside County (15+ homes), San Bernardino County (10+ homes), San Diego County (3 homes), and Los Angeles County (3 homes). Our main headquarters is located at 1215 Columbia Ave, Riverside, CA 92507.
We provide comprehensive services including 24/7 supervision, medication management, three meals daily, case management, transportation to medical appointments, basic wound care, health monitoring, life skills training, and connection to community resources. Specific services may vary based on individual care plans.
We provide medication management, vital signs monitoring, and basic wound care. However, we are not a medical facility and don’t provide acute medical care. We coordinate closely with residents’ healthcare providers and assist with attending medical appointments. For medical emergencies, we contact emergency services immediately.
Yes! We have fully ADA-compliant homes designed for residents with mobility challenges. These include wheelchair-accessible entrances, wide doorways and hallways, roll-in showers with grab bars, accessible fixtures, and ramps. We also have traditional two-story homes for residents who don’t require accessibility modifications.
Yes, we encourage family involvement and support. Visiting hours and policies vary by location and individual circumstances. We ask that visitors coordinate with staff and follow house rules to maintain a safe, therapeutic environment for all residents. Contact the specific home for visiting information.
Referrals can be made by calling our main line at 909-266-0218 (available 24/7), completing our online contact form, or having a healthcare facility contact us directly. Our intake team will review the referral, conduct an assessment, and determine the most appropriate placement and services.
Our intake process includes a comprehensive assessment of medical needs, behavioral health status, medication requirements, and personal goals. We review medical records, speak with referring providers, and meet with the individual (when possible) to ensure we can provide appropriate care. The process typically takes 24-48 hours.
Residents should bring personal clothing, toiletries, any current medications, important documents (ID, insurance cards, medical records), and personal items for comfort. Homes are furnished, so large furniture is not needed. We’ll provide a complete list during the admission process and can assist with obtaining necessary items if needed.
Yes, we accept referrals from behavioral health centers and work with residents who have mental health diagnoses. Our staff is trained in trauma-informed care and we coordinate with mental health providers. However, we assess each referral individually to ensure we can safely meet the person’s needs and that our environment is appropriate for their recovery.
Our case managers work with each resident to develop a comprehensive transition plan. This may include securing permanent housing, connecting to ongoing medical and mental health care, enrolling in benefits programs, finding employment or training opportunities, and establishing community support systems. We provide follow-up support to ensure successful transitions.
